Decoding the info on the form, the serial number and the production code finds the paperwork belonging to a 1953 Ford Customline 2 door sedan with the 239 V8 Flathead, scheduled for assembly in St. Paul on April 16, 1953, 14 days before delivery, finished in Woodsmoke Gray.

Looks to me like a custom 49 - 52 Mercury. Chopped top. B pillar removed. etc. Looks to be very well done.

I would suggest a late 1950s Hudson, based on what appears to be a full length body sculpture line.
